Welcome to the Stargate Training

Stargate (physician portal) is a web-based system that allows physicians access to their patient information from their office, home or one of our hospital facilities (via the physician computers).

How do I access Stargate? 
It's easy, just complete the required request forms, below and fax to Connie Reed at (210) 297-0911. Then, view the videos on this page (see step 2). Once you have viewed the videos please call Connie at (210) 297-9721 or contact her via email for your username and password. 

Doctors: Look for this symbol for quick access to your administrative tools through Stargate.
