Eligibility

• Patient’s must require more than one form of therapy three hours a day, five days a week
• Be medically stable and have a medical necessity for 24-hour nursing care
• Demonstrate sufficient endurance and potential to participate in rehab program
• Exhibit potential for making significant gains in functional capabilities
• Demonstrate willingness, along with their families, to be involved in a rehab program
• Show potential to return to this community

Referral

We welcome referrals from:

• Physicians
• Other health care professionals
• Discharge planners
• Case managers
• Family members
• Patients themselves

After receipt of referrals, our staff will arrange for and complete a pre-admission assessment at no charge. Tours can also be arranged.

Baptist Rehabilitation Centers accept patients from any location without a three-day qualifying stay. Physician’s offices, nursing homes, and other like entities may request evaluations.
